PREVENTIVE DENTISTRY

It is very important to form a habit of visiting your dentist regularly. By undergoing regular preventive dental exams, at least twice a year, it is possible to stop dental and gum diseases or discover them on time, so that you can immediately start your therapy. Although the modern dentistry has evolved a lot, it is still not discovered what the cause of tooth decay is. So the main goal of preventive dentistry is to prophylactically work on the occurrence of oral and dental diseases.

Within preventive dentistry, we are motivating and teaching our patients how to properly maintain oral hygiene, we are removing dental calculus, applying fissure sealants and making dental splints.

1) What you should know about oral hygiene

Regular and proper teeth cleaning are the basis of oral hygiene. In order to properly maintain oral hygiene, you need to:

a) Use adequate toothbrush

When choosing your toothbrush, you should have this in mind: toothbrush with the label ‘MEDIUM’ (medium firmness) is suitable for the largest number of patients, while the toothbrush with the label ‘SOFT’ or ‘ULTRA SOFT’ should be used by children or patients who suffer from parodontosis or have sensitive gums. Toothbrushes with the label ‘HARD’ should be used by patients with ceramic dental prosthesis.

Toothbrush should be replaced every three months. Nevertheless, patients with braces have to replace their toothbrush more regularly.

b) Use adequate toothpaste

d) Properly brush your teeth

Researches have proven that only a very few people clean their teeth properly. In order to clean your teeth the right way, you should do it for around 3 to 5 minutes. It is important to clean off everything from the teeth surface as well as from your tongue!

You should start brushing your teeth from the last one in the back to the front ones and you should start from the top and then continue to the bottom jaw. Fibers of your toothbrush should be positioned at the angle of 45 degress against your teeth.

You should wash your teeth upwards, in the direction of the growth of your teeth. Teeth should be brushed two times a day (in the morning and before going to bed).

The tongue should be cleaned from the root to the front. That way, the number of bacteria in oral cavity is lowered and the unpleasant breath is eliminated.

2) Removal of dental calculus and soft deposits

The removal of dental calculus and soft deposits is one of the most important dental interventions when it comes to maintaining oral hygiene, and overall oral health.

Dental calculus is formed by the mineralization of the dental plaque, while those minerals are formed in the saliva.

The process of the forming of dental calculus is individual. It is forming faster in older people, the people that have higher Ph of their saliva, as well as in smokers.

It can be supragingival (visible to the naked eye) and subgingival (invisible to the patients).

Supragingival calcuculus is formed near derivative canalas of salivary glands (in the bottom jaw behind incisor teeth and in the top jaw around molar teeth). Subgingival calculus is formed only in patients that have parodontal pockets and it can appear around any tooth.
Removal of dental calculus is extremely important since it causes inflammation of gingiva and unpleasant breath.

The removal of dental calculus is completely painless and is performed with an ultrasound and instuments that are specifically made for this intervention. Some patients might have an unpleasant feeling during this procedure.

After the removal of the calculus, it is important to polish the surface of the teeth. It is recommended to remove calculus at least twice a year. Smokers should do it even more often.

3) Applying dental fissure sealants

Applying dental fissures sealants is a prophylactic method in dentistry that consists of covering dental surfaces in order to stop the occurance and the development of tooth decay.
The procedure is completely painless, which is why it is very well-recived in younger paitents.

Sealants are usually applied to fissures of the first permanent molar teeth, having in mind that those teeth have very important role in the further forming of the occlusion. Dental fissure sealants can be applied on other teeth, as well as on deciduous teeth if needed to.

Occlusal splints for athletes

Occlusal splints

Bruxism is a professional term for teeth creaking and crunching that is not related to the chewing of the food. This occurs unconsciously, while the patient is sleeping, and the abrasion of the chewing surfaces forms as a result of that. In time, this occurance can lead to the opening of the dental nerve, changes in the jaw joint and the pain in the jaw, ears and head, which leads to a big tension and nervousness in patients.

Protective splint should be worn only during the night, while the patient is sleeping. It is important to mention that by wearing the splint you can’t cure bruxism, but only alleviate consequences of that and stop new negative occurrencies on teeth and jaw.

Orthodontic retainers

After finishing the orthodontic therapy (after removing the braces) it is necessary and important to stabilize the newly-formed teeth and jaw positions, which can be done by wearing orthodontic retainers.

PARODONTOLOGY AND LASER THERAPY

Periodontitis is the most widespreaded disease of the modern world. The last stadium of this disease is a complete loss of the teeth.

Besides taking care of the teeth hygiene (and the removal of calculus), one of the most successful methods of prevention and curing of this disease is a laser therapy. We are using this method for years and it has been giving great results. The treatment is completely painless.

CONSERVATIVE DENTISTRY

Conservative dentistry is a branch of dentistry that is engaged in the recovery of teeth affected by cavity while using minimally invasive methods.

Tooth decay or cavity is a disease of a hard substance of the tooth. It can be fixed by using a mechanical removal of the part of the tooth that is softened by the cavity and then putting a dental filling inside of the hollow. While doing that, it is important for a dentist to preveserve the natural look of the patient’s tooth.

Besides sanitation of tooth decay, conservative dentistry includes healing of teeth – endodontics. If the tooth decay is not sanitated on time, the infection will most likely develop and that will lead to an inflammation of a dental pulp – pulpitis. It is extremely painful for paitents, which is the reason why they are forced to visit a dentist.

Curing treatment is a procedure of a removal of an infected content from the pulp chamber. After that, the removed part of the tooth is compensated with a dental filling.

What is important for the patients to know is that the teeth that have a pulp removed have much lower biological potential which is why they are more prone to fractures.

For filling and healing the tooth and its root, we are using top-quality imported materials and medications of the latest generation. Teeth whitening

Very large number of people is unhappy with the color of their teeth. Yes, the colour of our teeth is a hereditary factor, but it can be influenced by: pigments from some food or beverages (coffe, tea, red wine, Coca-Cola, etc) or bad habits like smoking, some medications (tetracyclines that pregnant women and children should avoid), as well as other diseases.

There are two methods used in teeth whitening:

1. in-office teeth whitening – the advantage of this method is the speed of achieving visible results, as well as the professional control of the procedure. Before applying the product, it is important to protect the gums. The whole treatment can be repeated in the next few days (depending on the shade you’d like to achieve). After this procedure, your teeth will be more sensitive, but the degree of the pain varies from person to person, as well as the effect of teeth whitening. 2. At-home (or nighttime) teeth whitening – this is a very popular method. After making your unique splint for teeth whitening, you will fill it in with a whitening gel by yourself and then apply it on your teeth. Both methods are using the same products (hydrogen peroxide or carbamide peroxide). The difference is in the concentration of the product. Dentist is the one who should estimate which method is suitable for each patient. It is also important to accentuate that teeth whitening is a complex intervention and that is shouldn’t be performed without the professional supervision.

Avital tooth whiteninga

After the endodonthic teeth treatment (extraction of the tooth nerve), a tooth, in time, changes its colour and starts to darken. To whiten this tooth, there are two methods:

1. By using the high concentration of the whitening gel, while protecting the other teeth and gums

2. By using so called “wandering techinque” – when the dentist applies the gel inside of the tooth and closes it with a temporal filling. Like that, the gel should stay in the tooth for 7 days. If necessary, the treatment can be repeated. Permanent filling is applied 10 days after finishing the treatment.

Composite bevels and porcelain veneers

In some cases, dental tissue is so damaged that the making of cosmetic filling is almost impossible and the whitening procedure isn’t giving expected results. One of the ways of solving this type of problems is the production of composite bevels or porcelain veneers that fulfill high aesthetic expectations.

Bevels can be done during one visit to the dentist. After doing the preparative drilling of the tooth, the material is applied directly on the drilled part and is modeled that way. Bevels are much cheaper and the aesthetic effect is good, but their lifetime is limited.

Porcelain veneers are made similar to the composite bevels. After drilling the front part of the tooth, we take the print of the tooth in that condition and then we produce veneers in the laboratory. Cementing of the veneers is then performed with the special type of cement. Veneers are long-lasting and give excellent aesthetic effect.

DENTAL PROSTHODONTICS

Prosthodontics is a part of dentistry that is focused on the recovery of the physiological shape of the teeth and the tooth line, of the teeth that are damaged by the loss of dental tissue or that have fallen out, or if the teeth are damaged because of the cavities, trauma, abrasion or irregularities in the forming of the dental structures.

Besides the aesthetic deficiency, functiones of orofacial system are endangered as well, while chewing, talking, breathing, and swallowing becomes more difficult. The patient can’t properly chew his bite, which can lead to systematic disorders.

Due to the lack of some teeth, the rest of them can start to dangle or move to the empty space, which can lead to the damage of the upper and the bottom teeth ratio, as well as making prostetic care more difficult. That is why it is needed to compensate the lack of the tooth, or tooth line. Prolongating the prostetic care leads to the loss of the size of the bones and the appearance of the disorders in temporomandibular joint.

Production of metal ceramic crowns and bridges

Bridges and crowns are fixed prosthodontics’ work that are cemented on the rest of the teeth and are then attached in the mouth of the patient. The period of adapation and habituation is very short.

Adequatelly made dental crown will surely elongate the life of the tooth. Edentation is the indication for the production of the bridges, under certain condition (that the gaps between the teeth are not too big, that the future teeth, the carriers, are in a good shape, as well as their support apparatus).

The procedure implies detailed examination and analysis of the condition of oral cavity and the remaining teeth, as well as a recovery of all disease which represents pre-prosthodontics preparation.

After that, the teeth, the carriers, are grinded in a local anesthesia, which makes the procedure painless, and then the imprint is taken. Making of non-metal dental crowns and bridges

Unlike in metal ceramic crowns where the gray line occurs in time, just by the edge of the gums (because they are withdrawing), it won’t appear in non-metal ones.
Due to the biocompatibility of this material, gingiva will strive to fill in the gap in between the teeth and your smile will get the youthful look and your whole face will glow.

Production of the combined work

Sometimes, it is impossible to produce high-quality fixed work because the number and the disposition of the remaining teeth are not satisfactory. The therapy for this type of edentation requires a combination of fixed and mobile prosthesis.

Production of partial skeletal prosthesis

Partial skeletal prosthesis is the most reduced prosthesis whose base consists of the metal skeleton. The degree of reduction is different depending on the number and the disposition of the remaining teeth as well as the condition of their supportive tissues. It is made as a permanent compensation for almost all kinds of edentation. This type of prosthetis is mobile, which means it is not fixed to the teeth and can be taken off for the proper maintenance of the oral hygiene.

Production of a partial denture

Partial plated denture is indicated therapy for all types of edentation. Nevertheless, it represents only temporary therapeutic solution, as a guardian of the space until the more permanent prosthesis is done. It consists of the denture, made of acrylate, artificial teeth and retentive elements (wire hook) by whome it is then hanged on the rest of the teeth.

Partial denture only in rare cases can be a permanent solution. Those cases are: when the patient has a small number of teeth, supportive tissue of the remaining teeth are not capable of accepting additional burden or due to the lack of financial assets of the patient for covering the expenses of the production of permanent prosthesis.

Production of a complete dental prosthesis

The loss of all teeth brings various problems with itself. Above all, chewing is disabled and with that normal digestion is disabled as well, which is why there can occure various problems in the digestive system, and which can affect overall health of the patient. Heavy breathing might occur as well.

The disorder in the relation between jaws, which is followed by the symptom of the teeth loss, leads to changes on the jaw joint. Asthetics in patients that suffer from edentation is seriously corrupt, the bottom third part of the face is getting smaller, the corner of the mouth starts to fall down and nasolabial wrinkles are getting deeper.

Besides health and aesthetic problems, it is proven that a complete loss of teeth leads to a various psychological problems (from isolation to deep depression).

Complete dental prosthesis is indicated therapy in patients that suffer from edentation. It should compensate for the lost teeth and for the part of alveolar ridge and enable renewal of corrupt aesthetics and functions.
Production of a good quality complete prosthesis requires a professional knowledge and experience of the doctor as well as of the dental technitians that have the task to completely reconstruct the teeth of the patient and to help him get used to this change as fast and easy as possible.

One of the major problems that patients face in the case of complete prosthesis is the acceptance and adaptation to the prosthesis itself. Adaptation means that your body stops to notice stimulation and the patients need a certain amount of time to be persistant and to adapt to the foreign body in their mouth. Problems that the patient can face in the beginning are: the prosthesis starts to pinch and the patient feels discomfort or possibly the pain, the prosthesis isn’t stable (especially the bottom one) because of the functions of the tongue, the patient eats and speaks with difficulty and has the feeling that the prosthesis is too big. He can also have the urge for vomiting.

All these problems will disappear in the period of two weeks. In that period the dentist will make corrections of the prosthesis several times.

Production of “flippers”

The flipper represents a temporary prosthodontic solution for compensation of one missing tooth.
Flipper has the purpose of a space keeper for the future permanent prosthesis. It represents satisfactory aesthetic, but not the most functional solution.
